2026 Performer & Instructor Application Form

Thank you for your interest in participating in the 16th Annual Dance for World Community Festival, Greater Boston’s largest and famously inclusive dance event.

Please fill out the form below to apply for participation in the Festival as a Performance Group and/or Instructor.

It is recommended that performing artists apply as early as possible. Capacity is limited to approximately 60 performance groups and instructors, and sufficient lead time will be necessary for application reviews, scheduling and stage assignments.

Applications must be submitted by the final deadline of Thursday, April 2, 2026. “Early Bird” applicants who submit by March 12, 2026 will be considered first. Applicants will be notified of their acceptances on a rolling basis, with final notifications on Wednesday, April 15, 2026.

FESTIVAL PARTICIPATION AGREEMENT

I am aware that dance and the exercises associated with it can place stresses on the body and can risk physical injury. On behalf of the above named organization and myself, I assume this risk and agree that José Mateo Ballet Theatre will not be liable in any way for injuries sustained during participation in the event or related activities.

I grant José Mateo Ballet Theatre the right to make and use photographic and audiovisual documentation of me to be used for philanthropic, promotional, and other mission related purposes. (In no case will your identity be disclosed without written consent.)

I understand that the primary method of communication from José Mateo Ballet Theatre is email through Constant Contact or Gmail in order to send announcements and other pertinent event information.
