FESTIVAL PARTICIPATION AGREEMENT

I am aware that dance and the exercises associated with it can place stresses on the body and can risk physical injury. On behalf of the above named organization and myself, I assume this risk and agree that José Mateo Ballet Theatre will not be liable in any way for injuries sustained during participation in the event or related activities.

I have read and agree to comply with all requirements stated in the application. I understand that a payment will be required to confirm my participation. Payments can be made online. I understand that if I fail any City of Cambridge inspection I may not be able to operate and I will not qualify for a refund. I understand that in order to complete the application, I will also need to apply for a Temporary Food Service Permit through the City of Cambridge. You will be redirected to this link upon completion of this form.

Additionally, please note, this is an environmentally friendly event. Prohibited items include styrofoam, plastic bottles, and non-recyclable plates, utensils, and napkins etc.

I grant José Mateo Ballet Theatre the right to make and use photographic and audiovisual documentation of me to be used for philanthropic, promotional, and other mission related purposes. (In no case will your identity be disclosed without written consent.)

I understand that the primary method of communication from José Mateo Ballet Theatre is email through Constant Contact or Gmail in order to send announcements and other pertinent event information.

This year, we are asking Food Vendors to provide a free food item of your choice to DWC Festival Staff and Volunteers.

DWC Festival Staff and Volunteers will each receive one food voucher that they can redeem at any festival food vendor during the course of the festival.
